Buckinghamshire Healthcare Trust (who own the car park at Thame Health Centre) are introducing charges on MONDAY 14TH OCTOBER. The Rycote Practice has negotiated that the first hour will remain free but you will need to pay after this. Please note that this decision had NOT been made by The Rycote Practice and we will receive no parking funds resulting from this decision.

The Rycote Practice will be closed from 1pm on Tuesday 15th October for essential staff training. We apologise in advance for any inconvenience.

The Rycote Practice welcomes all new patients within its catchment area. In order to register you simply need to print and complete the new patient questionnaire (below) and bring it into the practice along with proof of ID. Please also ensure that you print and complete the text messaging consent form (if required) and also the Summary Care Record and Oxfordshire Care Summary (mandatory) at the bottom of this page.

We would also strongly encourage you to complete a text messaging consent form. This will ensure that you get a text message confirming your appointments at the time of booking and a reminder text approximately two days before your appointment. Please bring this with you when you submit your new patient questionnaire.

We also ask you to complete the form below that confirms your preferences with regard to the national Summary Care Record (SCR) and the local Oxfordshire Care Summary (OCS).

The SCR makes your current medications and allergies available for other NHS staff to ensure that your are treated safely. Your details will be automically uploaded unless you opt out. You can also agree with your GP to have additional significant medical information uploaded to your SCR if you wish.

The OCS makes more detailed information including significant diagnoses, test results, vaccinations, appointments, referral letters and discharge summaries available for healthcare staff across Oxfordshire to view to help them care for you appropriately. Again, your details will automatically be uploaded unless you opt out.

The form below allows you to express your preferences for both the SCR and the OCS on the same form. Please submit this form along with your new patient questionnaire. NB - neither of these two schemes should be confused the with the Care Data project. More information on this information sharing intiative can be found here.
